The five levels of care, without the acronym soup.

Treatment isn't one thing — it's a ladder. Pick the wrong rung and you'll either drop out (too intense) or relapse (not intense enough). Here's how it actually works.

Medical Detox

Supervised withdrawal — 24/7 medical staff to keep you safe coming off the substance.

Best fit
You're physically dependent (alcohol, benzos, opioids) and stopping at home is dangerous or has been before.
How long
3 to 7 days, sometimes 10. Detox is a launchpad, not a destination — it almost always leads into residential or PHP.
A typical day
Vital signs every few hours. Medication to ease withdrawal symptoms (Librium, Suboxone, etc.). Mostly rest. Light therapy intake near the end.

Residential / Inpatient

You live at the facility 24/7 for the duration of the program.

Best fit
Home is a trigger, you've relapsed at lower levels, or this is your first treatment and you want full focus without distraction.
How long
28, 60, or 90 days. Some long-term programs go 6–12 months.
A typical day
Wake at 7. Group therapy, individual therapy, recreation, family sessions, 12-step or alternative meetings, structured downtime, lights out around 10. Phones are usually limited, especially in the first week.

Partial Hospitalization (PHP)

Day program: full clinical hours, but you sleep at home or in sober living.

Best fit
You're past detox and stable, but not ready to handle real life yet. Often the step down from residential.
How long
5–6 days a week for 2–6 weeks.
A typical day
9am–3pm of group, individual therapy, psychiatry check-ins. Evenings and weekends free for sober living, family, or aftercare meetings.

Intensive Outpatient (IOP)

Real clinical hours that fit around a job and a home life.

Best fit
You can stay sober between sessions, you have stable housing, and you need structure without dropping out of life.
How long
3 days a week, 3 hours each, for 8–12 weeks.
A typical day
Show up after work or in the morning. Group therapy, skills building (CBT/DBT), one individual session a week, urine screens. Go home, go to a meeting, sleep.

Standard Outpatient

Weekly therapy plus a group or two. The maintenance level.

Best fit
Step-down from IOP, or a milder case caught early, or long-term relapse prevention.
How long
Ongoing — months to years.
A typical day
One 1-hour individual session and one 1.5-hour group per week. Otherwise, you're back in your normal routine.
